Description
I've attached two pictures of my application: designer1.png and designer2.png

The first is in the BDS toolbar designer - you can see the last two icons on the toolbar are blank.

The second is the application running - you can see the last two icons aren't blank.

I got this after dragging a button I'd just created to a different position within the toolbar. All icons after it then showed blank, until I did the same again.
